A Case of Riedel's Thyroiditis

Riedel's thyroiditis is a rare variant of thyroiditis and characterized by the replacement of thyroid parenchyma by extensive fibrosis. Typically, the thyroid gland is diffusely involved and a painless, hard anterior neck mass shows clinical features similar to those of anaplastic thyroid carcinoma: a rapidly enlarging, hard, fixed thyroid mass and symptoms such as dysphagia, dysphonia, and dyspnea. Riedel's thyroiditis is associated with hypothyroidism, hypoparathyroidism, compression of the trachea, larynx, carotid sheath and esophagus. It may also cause invasion of adjacent muscles and mediastinum. Riedel's thyroiditis is also associated with other fibrous inflammatory processes, including retroperitoneal fibrosis, orbital pseudotumor, mediastinal fibrosis, sclerosing cholangitis and fibrosis in other organ systems. We report a rare case of Riedel's thyroiditis that presented a diagnostic dilemma but had a good response to high dose of steroids.
