Efficacy of Newly Developed Dermal Matrix Substitutes on Various Full Thickness Skin Defect

Introduction
For the dermal replacement for full thickness skin defect, there are several options to use biotechnologically derived extracellular matrix components or tissue scaffolds of cadaver skin origin. In this study we introduce a new dermal substitute derived from cadaver skin and present the surgical outcomes.(Materials and Method) We have retrospectively reviewed the medical record of 23 patients who were grafted with Acellular dermal matrix and thin autologous S.T.S.G. from June, 2010 to May, 2011. We reviewed graft survival and evaluate postoperative functional skin analysis. In a clinical trial, an individual comparison was performed between the acellular dermal matrix graft with split-thickness autograft and a surrounding normal skin. At the same time we compared CGDerm graft group (n=15) with Alloderm graft group (n=8) with using the Cutometer, Corneometer, Tewameter, Mexameter for objective measurements and Vancouver scar index (VSI) used for subjective measurements.

Results
The average graft take rate of new dermal substitute was 90%. Among 15 patients who underwent CGDerm graft, 4 patients resulted partial graft loss and all healing well with secondary healing. Comparing CGDerm grafted skin to surround normal skin, Mean elasticity, Hydration and skin barrier walues were 87%, 86% and 82% reported. AlloDerm grafted skin values were were 81%, 84% and 82%. There were no statistical difference between CGDerm and AlloDerm group at graft survival rate and skin analusis values. The CGDerm grafted group showed better subjective scar index (Vancouver Scar Index) than the surrounded scar area.

Conclusion
New dermal substitute (CGDerm) demonstrated comparable result at elasticity, humidification, skin barrier compared with conventional dermal substitute. We suggest that the use of Acellular Dermal Matrix Allograft (CGDerm) for good alternative dermal substitution in reconstruction surgery.
